Team Jerseys will be provided by Kerala Samajam

Forms and Payment
Tournament cost: $300 paid online through Pay Pal by December 9, 2015
Registration Form: Emailed to k.s.youth13@gmail.com
Waver Form: MUST be filled out and brought on tournament day by each player to participate. 

Registration Deadline: October 31st, 2014

 

A LATE FEE WILL BE ADDED TO YOUR TOTAL IF NOT PAID BY

DECEMBER 9, 2015

 

How to Register:

 

1. Download and complete the registration form .

2. Email the completed forms and an attached copy of photo ID to k.s.youth13@gmail.com 

3. Once all rosters and payments are approved, a confirmation email will be sent to the team captain.

4. Read The Game and Tournament rules.

5. Download and print the waiver form.

6. Fill out waiver form and bring it on the tournament day to participate.

7. Bring a photo copy of ID to participate.
